The following are steps that need to be taken to receive your clearance. All completed forms should be printed and handed into Gail Miller - SHAA Secretary. Please note that some of these links are outdated. The links will get you to the website, you need to locate the current 2014/15 version. Thanks.
1. You will need to take the online course "Creating a Safe Environment Training". The website to go to is: https://safeyouth.hbgdiocese.org/
2. Print out your results.
3. Go to the Pennsylvania State Police website at https://epatch.state.pa.us/, select record check, and new record. Your results are instantly provided to you. Once you get the results select "certificate" at the bottom and print out your clearance.
4. Print and complete the Diocesan Disclosure statement from the following link: http://www.hbgdiocese.org/wp-content/uploads/2012/05/Employee-Volunteer-Information-Disclosure-Statement-English-revised-Feb-2012.pdf
5. Next you will need to go to http://www.dpw.state.pa.us/ucmprd/groups/webcontent/documents/form/s_001762.pdf for the Pennsylvania Child Abuse Clearance. You must complete the entire form, include a $10 money ordered payable to the Department of Public Welfare and mail to CHILDLINE AND ABUSE REGISTRY, DEPARTMENT OF PUBLIC WELFARE, P.O. BOX 8170 HARRISBURG, PA 17105-8170
6.Next you must register as a coach at the following website: https://www.leaguelineup.com/FormPreview.asp?sid=926519906&url=hbgcyo&FormID=65189.
7. Any new coach must attend one of the three orientation dates before you are able to coach in a CYO program. Topics of the orientation will include what it means to coach or Minister to young people through athletics, legal issues, organization or time management and ideas for getting the most out of young athletes. While all new coaches must attend, any CYO coach is invited to attend. Either way, there is a $10.00 fee and pre-registration is required.
